IIf you prefer to complete this form offline,
you may download it here and mail, fax, or deliver the
completed form to us.

Certificate of Adoption
This form should be completed and the original
returned to us for submission to the Bureau of Vital
Statistics upon completion of your adoption process.
This form should be accompanied by a personal check
payable to the "Bureau of Vital Statistics"
for children born in Texas, or to the appropriate state
department for children born outside Texas. Please check
with us on the amounts and name of the proper agency
for payment. Please be very careful when you complete
this form. In addition to accuracy, it is imperative
that the form be legible and without any corrections
or 'white out.' This is the form that will generate
your new birth certificate.

This form will be e-mailed to you upon receipt
of your completed questionnaire. Upon receipt, you should
sign it and return it to us via fax or regular mail.
Your payment should also be made at this time. You may
make payment by check or money order accompanying the
signed contract, or by using the link below to make
a payment online.

Authorization for direct payment
If you have been told by your caseworker that
you are eligible for subsidies and reimbursement of
non-recurring expenses, you should print out this form,
sign it and return it to us to enable us to bill the
State directly for your adoption fees and expenses.

Release of Infant to 3rd Party
Hospitals will usually accept this form if properly
completed and executed, to release a child to the prospective
adoptive parents after birth.
